hey Ian,
The last time I went to the NAC (or was it MDA) and filled up the form, but you need the NRIC and full name of the bands' members, plus you need to ensure all lyrics are clean, and no copyrighted material. Worst come to worst go and call your OMS seniors up. All the best with Stasis, it looks like a good bill this year.
